This page explains, in plain language, what information the DFO Health physician questionnaire collects, how it's used, and how it's protected. If anything here is unclear, email us — we're glad to explain further.

What We Collect

The questionnaire asks for your name, practice name, and specialty, along with your answers about how your practice operates day to day. Your email and mobile phone number are optional — we only ask for them in case you'd like us to follow up with you directly.

Participation Is Voluntary

Completing the questionnaire is entirely voluntary. You may skip any optional question, and you may stop at any point without providing a reason.

Your Responses Are Confidential

We treat every response as confidential. Individual responses are reviewed internally by DFO Health and are never published, sold, or shared publicly without your explicit permission.

How We Use Your Responses

Responses are used for research purposes — to understand the operational challenges independent practices face, and to inform how DFO Health approaches future product development. We do not use your responses for advertising, and completing the questionnaire is never a sales pitch.

Please Don't Include Patient Information

This questionnaire is about how your practice operates — not about individual patients. Please do not include patient names, medical record numbers, or any other protected health information in your answers.

How Your Information Is Protected

Access to your responses is limited to named, authorized DFO Health team members — no one else can see them. Responses are currently collected using Tally and stored in Google Sheets; we do not share your responses with any other third party.

If You Share Contact Information

If you provide your email or phone number, we will only use it to follow up with you directly about a possible conversation or pilot program — and only if you told us that's okay in the questionnaire.

Correcting or Deleting Your Response

If you'd like to correct, update, or delete your response at any time, email us at hello@dfohealth.com and we'll take care of it directly.
